Massive Transfusion Protocol

Goals of an MTP are:

1. Identify possible or proven massive bleeding early
2. Optimise oxygen delivery to tissues
3. Stop the bleeding (IR or surgery)

Started By:

1. Senior clinician
2. Inform blood bank

Give: 1 (or 2):1:1 rbc:plts:ffp

Testing: Every 30-60 mins during active transfusion OR Every 4-6 units FBE, COAGS, GAS AIM: Hb > 70 Plts > 50

pH > 7.2 Lactate < 4 iCa > 1.1

INR < 1.5 Fibronogen >1.5 APTT < 1.5x Upper Limit

Always arrange for definitive treatment.
